SERPRO Checklist Resource
Use this template to record what happened during a spill, what was affected, what was used, who responded and what follow-up action is required. Good records help improve future spill preparedness.
This template is for supervisors, safety leads, facilities teams, warehouse managers, transport managers and anyone who needs to record spill events or near misses.
Use it after a spill, leak, near miss, response drill or internal review, especially where absorbents were used, products need replacing or site controls need improving.
| Section | What to check | Why it matters |
|---|---|---|
| Incident details | Record date, time, location, weather if relevant and who reported the spill. | Clear details help reconstruct what happened. |
| Substance | Record the liquid or chemical name, estimated quantity and whether SDS information was consulted. | The substance affects clean-up, exposure and disposal decisions. |
| Source | Identify the cause, such as damaged container, overfill, vehicle leak, hose failure or unknown source. | Understanding cause helps prevent recurrence. |
| Immediate actions | Record isolation, drain protection, containment, clean-up and escalation actions. | This shows how the response was managed. |
| Products used | List pads, socks, booms, drain covers, PPE, bags, trays or neutralisers used. | Used items need restocking and may indicate kit suitability. |
| Waste handling | Record how contaminated absorbents and waste were contained and passed for disposal. | Waste records may be required by site procedure. |
| Impact | Note affected surfaces, drains, stock, equipment, vehicles or people. | Impact helps determine follow-up action. |
| Lessons learned | Record changes needed to equipment, storage, training, signage or inspection routines. | Documentation should lead to improvement, not just filing. |

This checklist is provided as general practical guidance only. It does not replace a site-specific risk assessment, COSHH assessment, safety data sheet review, environmental advice, legal advice, emergency procedure or the instructions of a competent person. Always assess the actual substance, quantity, location, drainage route, exposure risk and site controls before selecting spill response equipment or carrying out clean-up work.
